Available Assessment/Surveys

The following surveys have been designed and tested by research scientists and are made available for use by HIV prevention program planners and designers. Instruments included on this site come from CHIPTS investigators as well as outside scientists, they are provided for educational purposes only. If you decide to use or modify any of these instruments it is the user’s responsibility to insure all copyright and use issues are met. Contact information is provided where possible to inquire about any fees or use restrictions.

Warning: Survey needs vary from population to population and project to project. Please consider carefully the needs of your particular intervention before using one of these instruments.

We encourage researchers and service providers to use these instruments, adapting and modifying as needed for your population. Compliance with copyright restrictions and use fees are the user’s responsibility. There are instruments included from CHIPTS research scientists, for those, please cite CHIPTS as the source in any presented or published work, and, if the instruments were modified in any way from their original (as downloaded from the CHIPTS Web site) please note that in any presentations.
